Indemnity Waiver and Release of Liability

Being permitted by BELLY RUB N GRUB 501(c)3 animal rescue to participate as a volunteer, requires that this form be completed and signed, by each applicant. Thank you in advance for your cooperation. We look forward to reviewing your application.

BELLY RUB N GRUB 501(c)3 animal rescue. In consideration for being permitted by BELLY RUB N GRUB 501(c)3 animal rescue to participate as a volunteer, I hereby agree that I will assume full responsibility for the death, personal injury or property damage suffered or sustained me/minor child as a result of or in connection with my/minor child's participation as a volunteer. I agree, therefore, to indemnify BELLY RUB N GRUB 501(c)3 animal rescue from and against all liability, claims, demands, damages, judgments and costs for or arising from the death or personal injury of myself/minor child or damage to the my property suffered or sustained by me/minor child, as a direct or indirect result of the my/minor child's participation as a volunteer; even if such death, personal injury or property damage arises out of the negligence of BELLY RUB N GRUB 501(c)3 animal rescue (or any of its employees, volunteers, fosters or officers).

I further agree that I will not make any claim against, sue, attach the property of, or prosecute BELLY RUB N GRUB 501(c)3 animal rescue (or any of its employees, volunteers or officers) for any death, personal injury or property damage, whatever the cause or place of the event giving rise to the claim, which I/minor child may suffer or sustain as a result of or in a connection with my participation as a volunteer. In addition, I hereby release and discharge BELLY RUB N GRUB 501(c)3 animal rescue (and its employees, volunteers, fosters or officers) from all actions, claims or demands that I/minor now have or may hereafter have for any death, personal injury to myself/minor child, or property damage arising out of or in connection with my/minor child's participation as a volunteer. This release of liability is intended to discharge in advance, BELLY RUB N GRUB 501(c)3 animal rescue (and its employees, volunteers, fosters or officers) from and against any and all liability to me or to any person claiming under, through or on behalf of either of us arising out of or connected in any way with my/minor child's participation as a volunteer; even though that liability may arise out of negligence on the part of BELLY RUB N GRUB 501(c)3 animal rescue (and its employees, volunteers or officers).

I further understand that the behavior of domestic animals is sometimes unpredictable and that some domestic animals are capable of inflicting serious personal injury or death, as well as extensive property damage. Knowing the risks of handling domestic animals; nevertheless, I hereby agree to assume those risks and to release, indemnify and hold harmless BELLY RUB N GRUB 501(c)3 animal rescue and the persons mentioned above who might otherwise be liable to me or my heirs or assigns for damages. It is understood and agreed that this Indemnity Waiver and Release of Liability is intended to be binding on me/minor child and my heirs, distributes, legal representatives or assigns. I represent that I am over eighteen (18) years of age.
